
While GLP-1 receptor agonists like semaglutide and tirzepatide are growing in popularity, new research that for most, bariatric surgery remains more effective for lasting weight loss. In fact, a recent study that set the two methods head-to-head found that bariatric surgery resulted in five times more weight loss than GLP-1s.
Study Findings
Researchers from NYU Langone Health and NYC Health + Hospitals analyzed the medical records of real patients over the course of six years (2018-2024). They discovered that after two years, those who underwent sleeve gastrectomy or gastric bypass lost an average of 58 pounds, while those who had a GLP-1 prescription for at least six months lost an average of 12 pounds (24% vs. 4.7% total weight loss). Although patients with year-long prescriptions lost more weight than those with six-month prescriptions, they trailed significantly behind bariatric surgery patients with 7% total weight loss.
The Importance of GLP-1 Treatment Compliance
Interestingly, the patients on GLP-1’s in this study lost less weight than those in the drugs’ clinical trials (15% to 21% total weight loss). Our very own Rod McKinlay, MD, FACS, says this is likely because the study reflects a more real-world look at the results of GLP-1s. Clinical trial patients are followed up on much more closely and are seen through to maximum dosing, allowing researchers to record much more substantial results.
Dr. McKinlay says GLP-1 prescribers can learn from this discrepancy. “To me, one of the take-homes from this is how important it is to have good follow-up and compliance with the medications, which we can provide in our office with our great [nurse practitioners],” he notes.
